 COMMUNITY SUPPORT WORKER
Seeking a Community Support Worker/Case Manager to work with children, adolescents, and their families to assist in meeting the youth’s mental health treatment and service coordination needs. Requires provision of home-based assessments and interventions linking children with appropriate community-based services. Applicants with prior experience with minority youth and families desired. AA degree or higher in a mental health related field. Prior experience in case management services preferred. Must have reliable transportation. Flexibility in schedule a must.
